Policies and Forms

At Lotus Organics Day Spa & Wellness, it is important that the environment be as serene and comfortable as possible for both the client and the practitioner. In order to maintain the safety and comfort of all, clients are asked to familiarize themselves with the following Policies, and print and fill out the forms requested by the practitioner. Your cooperation is greatly appreciated!

All clients must read and sign the form(s) for the appropriate treatments, and read and accept the policies listed below. Please print the appropriate forms and bring them you your first appointment. LOTUS ORGANICS DAY SPA & WELLNESS POLICIES & CLIENT EXPECTATIONS

  • The therapist retains the right to refuse service to any suspicious persons, those appearing to be under the influence of any drugs or alcohol, or those appearing to have signs and symptoms of any illness or disorder that may contraindicate the treatment to be given. Please be respectful of the health of others and reschedule if you are sick or have knowingly been in contact with any contagious conditions!
  • Clients must give at least 48 hours notice of appointment cancellation and/or changes in desired service(s) in order to avoid a fee of up to 100% of the originally scheduled service amount at the discretion of the therapist. Please notify the therapist with full and honest disclosure as far in advance as possible when needing to change/cancel appointments. Confirmation texts/calls from the therapist are NOT required, but are merely done out of courtesy when time allows. Clients are expected to attend their appointments regardless of confirmation from the therapist. Please properly record your appointments on your calendar(s) upon scheduling, and feel free to verify with the therapist at any time. Last minute meetings, travel plans, work/school functions, lack of child care, etc. will not necessarily be exceptions from the cancellation fee. Please plan accordingly.
  • Last minute changes to a significantly lesser costing service without prior notification to the therapist may be charged the full amount of the service originally scheduled. Please inform the therapist in advance if you would like to change your service so that time allotment and financial expectations may be adjusted.
  • No-Show clients, clients regularly cancelling less than 48 hours before their appointment, and clients cancelling more than two consecutive times without sufficient notice may be billed for the entire service and/or required to pre-pay in order to be rescheduled.
  • Arriving late for an appointment may result in a shorter session at the full-price of the originally scheduled service.
  • Payments are preferred in cash, Venmo, or PayPal (Friends & Family). Credit/Debit cards may be used for totals over $20, but please be mindful of processing fees. Personal checks are no longer accepted. No traveler’s checks will be accepted. If using a Gift Certificate, proof of certificate must be shown at time of service. The therapist is not obligated to keep record of all gift certificates.
  • Full payment is always due at the time of service and client must verify that they have the proper form/amount of payment before the service begins.  All prices and fees are subject to change.  Client should always verify prices before beginning a service. Failure to pay in full may result in legal charges and unpaid balances will be sent to collections.
  • All services and products purchased, including gift certificates, are non-exchangeable & non-refundable. One free product sample is available per product, per customer.  Additional samples may be given for a $2 per sample fee.
  • Minors under the age of 18 receiving a service must be accompanied by a parent or legal guardian at all times.  No children are otherwise allowed during treatment sessions.
  • Clients should be prepared to make their session as relaxing as possible by muting all cell phones and electronics, as well as informing the therapist of any special accommodations needed, and clarifying all questions and concerns.
  • Clients should arrive hygienically prepared for their session.
  • All clients must complete and sign all required forms before services can begin.
  • All clients must inform the therapist at the start of every visit if they are under any new prescribed medical or over-the-counter oral or topical treatments, if they have any allergies or sensitivities, or have been diagnosed with any new illnesses or injuries. Medical clearance may be required before certain services can be performed.
  • Clients should agree to follow therapist’s advice for post-session care to achieve the best treatment results.
  • The therapist is not responsible for lost items or items left behind.
  • Any sexual advances, sounds, comments or innuendos, or rude or violent behavior will result in immediate dismissal and will be reported to legal authorities.
